Privacy Policy of the Kintsugi Foundation

  1. General Information
    The Kintsugi Foundation, headquartered at Brzozowa 104/114, 05-830 Walendów (hereinafter referred to as the “Foundation”), is the administrator of personal data processed in connection with its activities. The Foundation makes every effort to ensure that personal data is processed in accordance with applicable legal regulations, including the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (GDPR).

    • Educational Institution Application Form:
      Full name of the institution, address (street, building number, city/town, voivodeship, postal code, country), type of institution, number of students, details of the director (first name, last name, email, phone number), details of the project coordinator (first name, last name, email, phone number), additional information.

    • Local Government Application Form:
      Name of the local government unit, type of local government, voivodeship, county, municipality, details of the person submitting the application (first name, last name, email, phone number), position in the local government, content of the application.

    • Sponsors and Donors Form:
      Company name, company address (street, building number, city/town, postal code), details of the person submitting the application (first name, last name, email, phone number), type of support (financial/material/service), details of the support, additional remarks.

  3. Purposes and Legal Basis for Data Processing
    The Foundation processes personal data for the following purposes:

    • Implementation of statutory activities and the Blue Mental Health Box project (Article 6(1)(f) GDPR),

    • Contact with educational institutions, local governments, and sponsors (Article 6(1)(a) GDPR – consent),

    • Performance of contracts and legal obligations (Article 6(1)(b) and (c) GDPR),

    • Marketing and promotion of the Foundation’s activities (Article 6(1)(a) and (f) GDPR).

  4. Recipients of Personal Data
    Personal data may be shared with entities cooperating with the Foundation, such as:

    • IT and hosting service providers,

    • Public institutions, if required by law,

    • Entities cooperating in the implementation of the Foundation’s statutory objectives.

    Personal data will be stored for as long as necessary to fulfill the purposes for which it was collected, or until consent is withdrawn, if the processing is based on consent.
